for applications with Citizens Community Bank. You may print the
appropriate form and complete it, to submit a loan application to
our bank. Loan applications can not be completed or submitted on
this web site, however you may print the application and complete
it, for submission in person, by mail, by fax or dropping it off at
a convenient night depository. Please feel free to call us, if you

If you are applying, for a loan to purchase a home, refinance a home
or the loan is to be secured by your primary home or a second home,
please use the Residential Real Estate Application. All other loans
that are not for farm or business purposes should use the Credit
Application. Farm and business purpose loans should utilize the
Credit Authorization form and appropriate financial statements.

feel free, to call your nearest Citizens Community Bank office. Our

identifies each person who opens an account.
What this means for you: When you open an account, we will ask for
your name, address, date of birth, and other information that will
allow us to identify you. We may also ask to see your driver's
